I get entirely too many automated feedback requests. At least 3 times that of staff concern by email or phone (preferred). Perhaps a personal touch without the need for constant feedback could help develop stronger relationships that would allow you to get that feedback from paid caregivers. It's taking up a lot of my time to do surveys and check-ins that feels more like being a research animal. It is impersonal to do them constantly, but still necessary quarterly or in person from time to time to gauge progress
