In spite of its unassuming appearance it is amazing. I work standing up all day, have constant back pain and shoulder tension. The way I use it is in a recliner, pads are behind my head and neck, the unpadded part in my low back, and I can feel my back and neck unload instantly. When I finally pry myself out I can move around much more easily than I would have been able to otherwise. That's my evening routine now and I feel that by the time I get to bed I drop off right away a instead of flopping around trying to get to a comfortable spot.
