It secures itself around my waist easily, securely and fits nicely under my clothes. This belt allows me to go about my day while icing my back. When I was reading reviews deciding whether to purchase or not I saw someone mention they used this for anxiety. I agree that something about the cold ice belt helps my pain and anxiety when I wear it. When I first opened the package and saw the neon pink logo and the thin gel packs,I thought there was no way this was going to be worth it. “A must have for anyone with lower back pain. Great, wonderful, amazing!!! It has given me the relief I have been seeking. I have slipped discs in lower back multiple times, yet still want to remain active. These are just what I needed.They are easy to use (freeze then put in neoprene holder and strap on). That's it! Strap is adjustable and keeps gel pack in place. Very comfortable that I sometimes forget I am wearing it.Having two packs means there is always one available for those times where I over do it and need a cold compress for reliefPlease note that I only used them as cold compress.”

“I use this as a cold pack. I've used it a lot since buying a few months ago, for sciatica issues. Belt and gel packs have both held up beautifully. Belt is long enough to do a sort of angled hip wrap on a small-medium adult (40" hips), not just a straight waist wrap.Haven't used it as a heat back, so I can't speak to that part.”
